PMDC form III problems

I have the following problems in Form III. Please help!!
Problem 1. Question 7, Section vi: the space in the Subject column on the form is not enough to fit my subjects of each year in. What do you propose I do?
Problem 2. Question 8 states "Proof of registration in the state or country in which qualification was obtained entitling the applicant to right of practicing the art of medicine/dentistry or a certificate from a registering authority that qualification obtained are registrable in the country of origin". What am I supposed to answer in respect to this question?
Problem 3. Page 6, (A) Specimen of undertaking on stamp paper of rupees 10 for proper identity of foreign graduate. Am I supposed to fill this on the form as well as on the stamp paper or just fill it on the stamp paper only and attach it to ......

1. Just write small n major medical subjects
2. Ur degree atestd by mofa etc z the proof of ur registration there
3. Just fill in the stamp paper
4. Course syllabus including subjects of each semester and their credit hours is better. Otherwise what you have, have to be submitted, requesting for specific paper after you left your university is a hectic job, so just go with what you have unless mandatory otherwise. ......

Hey,
form iii, q2 and 3: The registration fee and the processing fee 1000 and 5000 Rs respectively are they both supposed to be paid via bank draft together in the name of PMDC islamabad and in which bank?? After that am I suppose to print 4 copies of the bank draft slip and attach it with each form and keep the original.? Should the photocopy of the bank draft slip also be attested??


q6 and 7: Am i suppose to attest both the university syllabus and course outline coz the course out line is 3pgs x 4copies x 50rmb pages and the syllabus is 180 pgs. So if i have to do that than i am screwed as i would be paying 180pgs x 4copies x 50rmb.??? ......

For registration fee and processing fee, make a draft of Rs. 10500 from any bank. You dont have to attach 4 original or photocopies. Just give the original one which u will get from the bank to them. Take out a photocopy of that draft to keep it with you as a record if you want to. And you dont have to attest that draft.

For University syllabus and course outline, take out four photocopies of your mark sheet and attest them. I dont know about your university but i got a mark sheet from my university with all the subjects for each semester and study hours written on it. So i used that one for the syllabus. ......

1- you can deposit on the bank counter inside the pmdc office, islamabad, and write the receipt number on the forms, they will assist you surely.
2- you just need a course outline , of one page, no need lengthy and detailed syllabus. If you are able to get one page outline (mostly in tabular form, sample > http://min.us/mRAVbwbUq#1o) thats best. otherwise attest what you have of 3 pages. ......

hey,
guys on form III i saw

Registration fee Rs 1000
Processing fee Rs 5000/ addition to rgistration fee.

This adds up to 6000

Please can you explain why 10500 Rs?

I am from lahore so i can get the affidavit from lahore court?
Can i pay the money in the lahore Sheikh zayid office? ......

Eligibility letter, which you get after being confirmed from you university, costs you 6000 RS...and you can use its photocopy for all the exams...and even, God forbid, for the next try if you don't clear this time....

and registration fee for the exam is 10,500 which will work for all the three parts... ......
