Help in Experience Cert Application for PRES Exemption

    Good Day.

    New member, searched the forum, no luck.

    I recently completed HJ.
    And acquired Full PMDC HJ Certification.

    Kindly assist in the "second" step: Acquiring an Experience Certificate from PMDC

    I have downloaded the form (IV i believe), and filled it.
    Now the form has multiple instructions and is very confusing.

    Should I get copies of my HJ certificates (both 6 months ones as issued from my hospital) and have them attested by both MS of hospital AND principal of my med school? And is it supposed to be done in triplicate? As in 6 pages in total?

    Or are the requirements three copies of an actual experience certificate typed and submitted to PMDC from my hospital's HJ branch (and does it also require Principal/ MS attestation).

    Basically I'm asking what the Experience Certificates are. How many are asked for. And whose gotta attest them.

    I am applying to Ireland on basis of PRES exemption.

    Welcome to the forum bro
    1. Your all HJ certificates or if there is a single certificate given by your hospital copies should be attested by the MS of that hospital OR by Principal of your Medical college.
    2. Yes, 3 photo copies of the HJ certificates attested separately by the SAME person (MS OR Principal)
    3. Experience Certificate is of your HJ certificate as you dont have experience other than that.

    TY kindly for your prompt and concise reply.

    I shall make copies of my HJ certificates have them attested. Then submit them to PMDC with copies of my hospital issued Certificate of Experience also attested.

    I will make sure only one person attests.

    Uh Oh.

    I just read on the last page. This is confusing.

    Any help would be appreciated.

    It says:
    "You have got your experience certificates issued by medical superintendent/in charge of the hospital countersigned by your principal/dean."

    I don't know which certificate they are referring to. The single certificate of experience which was issued by Hospital, or the two HJ certificates.

    Also, I think my Principal would mind signing 9 pages. He's a grumpy fellow.

    Im attaching the PMDC form for any who wish to view, use etc.

    There are multiple instructions, all are confusing.

    It's the certificate whichever you are going to submit to pmdc to get experience letter from pmdc.
    1. Get the copies of certificate attested by MS.
    2. Get counter signature by your principle or dean.

    Excuse me for asking, should I get all 9 pages done this way? What I mean to ask is whether "all" documents I am sending must be counter signed.

    If pmdc requires you to sign all documents then there is no other choice. Is that hard to get signed 9 photocopies?

    Full disclosure. I went to him last week. And he gave me the third degree. A real earful.
    I didn't realize that the documents were required to be in triplicate as that is mentioned vaguely in the middle of the silly form.

    To return... would be... siiiiiigh.
